Change of telephone number: If you change your telephone
or mobile number, please let the Surgery know. Equally,
you
must be aware that if you have put an embargo on anonymous calls
that includes the surgery, in that case we will be unable to contact you
and therefore it may be prudent to lift this embargo.
Change of address: Please let us know if you move. If you leave
our practice area you will need to re-register with a GP who covers your
new address. We need to know all the patients in your household's name,
as each person has a computer file that will need to be amended.
